Conditions of Treatment
Please be advised that completing preliminary health and insurance questionnaires, including calling or having documents sent to our office does not establish a physician-patient relationship with this practice. Dr. Hirai will review your health history and conduct an initial evaluation at your first office visit to determine whether you are a suitable candidate and whether the practice will accept you as a patient.
Wait Times
Please note that due to high volumes, wait times can be longer. At times, Dr. Hirai has to see patients at the hospital on an urgent or emergent basis. In addition, a patient's condition may require him or her to be added on to the office schedule, or a patient who presents to the office may unpredictably need a procedure done at the same time of the scheduled appointment, extending the visit time.

The Open Payments database is a federal tool used to search payments made by drug and device companies to physicians and teaching hospitals. It can be found at https://openpaymentsdata.cms.gov.
For informational purposes only, a link to the federal Centers for Medicare and Medicaid Services (CMS) Open Payments web page is provided above. The federal Physician Payments Sunshine Act requires that detailed information about payment and other payments of value worth over ten dollars ($10) from manufacturers of drugs, medical devices, and biologics to physicians and teaching hospitals be made available to the public.
